Please make sure that you arrive in our office at least 15 minutes prior to your scheduled appointment. We will need you to fill out a few simple forms and provide us with your insurance information so that we can enter you into our system. To save time, you can print the New Patient Forms located on our New Patient Forms, fill them out and bring them to your appointment.

No, you do not need a referral to make an appointment with either a primary physician or a physician assistant. We are currently accepting new patients.  However, prior to your appointment, we encourage you to contact your insurance provider so they can explain the terms of your policy. Request an Appointment

If you have health insurance, please present your membership card when you check in. Depending on the terms of your agreement, we will collect your co-pay or bill you through your insurance plan. If you do not have health insurance, we require payment at the time of service.  We strive to have all aspects of our practice, including our professional fees, reflect our ethical and rational approach to your care.
